Application to adopt a dog

Before completing this form, please read all of the questions first to ensure you have the necessary information as this form cannot be saved to be completed at a later date. If you have a garden you will need to provide photos showing the area/fencing.



    Important: If you live in rented property you will need to provide written permission from your Landlord to keep a dog. This may be included in your tenancy agreement. Without this your application cannot be considered.

    ‘Owned’ means you have been responsible for the care and control of the dog, and you are/were the person responsible for the veterinary fees. Please give full details of current dogs/previously owned dogs, starting with current or most recent pet.

    Important: If you own a dog now, or have owned a dog previously you will need to provide veterinary records. Without this your application cannot be considered.

    Important: Have you considered what will happen to your dog in the event of holidays, increased working hours, change of job, having children, relationship breakdowns, moving house, illness/incapacity, any other change of circumstances? The average life span of a dog can be 12-15 years. Please confirm you understand this and are committed to giving a dog a home for the rest of its natural life.


    Important: It is a condition of the adoption agreement that if your dog is not already neutered you will arrange with the staff at Cardiff Dogs Home for this to be done at the earliest opportunity. Neutering costs are included in the adoption fee. Please confirm you understand and agree to this condition.
